Birthday Off One Floating Holiday Position Summary For the term of
one year, The Community Health Worker (CHW) will provide care
coordination and patient support services for individuals with
changing and complex healthcare needs. The CHW will serve as a
liaison between patients, primary care providers, specialists, and
other members of the healthcare team to ensure effective
communication, continuity of care, and improved health outcomes.
The CHW will work closely with patients who require multiple or
ongoing medical services to assist in navigating the healthcare
system, understanding care plans, and addressing social or systemic
barriers to care. The CHW will aim to: Improve continuity and
coordination of care for patients with complex needs Increase
patient understanding of and adherence to care plans Reduce missed
appointments and preventable hospitalizations Enhance patient
satisfaction and trust in the healthcare system Support primary
care providers and specialists by closing communication gaps and
ensuring follow-through on care recommendations. Additionally:
Build and maintain partnerships with local organizations and other
stakeholders. Assist with monitoring program outcomes and generate
impact reports for internal and external stakeholders. Skills &
Competencies: Strong organizational, communication, and
problem-solving skills. Ability to assist patients with navigating
social determinants of health Culturally competent and committed to
health equity and community-based care. Education Minimum: High
School Diploma or GED. Preferred/Advanced: Associate's or
Bachelor's degree in Public Health, Social Work, or a related
field; Community Health Worker certification Experience & Skills:
Lived Experience: Personal experience with health challenges
(addiction, disability, immigration), shared culture, language, or
background is a huge asset. Work Experience: Previous roles as a
Community Health Worker, case manager, or volunteer in
health/social services. Experience working in or with community
health centers, or safety-net providers Proficiency in computer
skills including Word, Outlook, and Excel Must be able to work
