Responsibilities: - Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care - Collaborate with leadership to review care coordination initiatives - Develop intervention plans, and share ideas about network and risk management issues - Work alongside pharmacists on medication management - Implement an individualized patient care plans - Timely response in chart completions and pharmacy recommendations - Provide oversight and mentorship to our clinicians in the area Experience as a medical director in a skilled nursing facility preferred. However, clinicians who are board certified and experienced in the following disciplines are encouraged to apply: family medicine(FM), primary care, internal medicine (IM), emergency medicine (EM), adult, post-acute, home health, hospice, palliative, urgent care, and hospital medicine (HM).
