DescriptionJOB PURPOSE:The Palliative Care Bridge Nurse will assist the Medical Director and Palliative Care Nurse Practitioner in providing palliative patient support as per policy and regulation. -KEY RESPONSIBILITIES:1. Responsible for taking and processing Palliative Care referrals.2. Provide support to the Palliative Care Nurse Practitioner and Medical Director.3. Will schedule visits to support goals of care as ordered by MD.4. Coordinate and communicate patient care goals between all providers.5. Make field visits to evaluate appropriateness, determine eligibility for services, present the palliative program, obtain legal consents, and assess interdisciplinary needs.6. Ensure communication of assessment and reassessment findings to attending physician.7. Complete all documentation accurately, timely, and in accordance with company policy and procedure.8. Inform palliative team of all new admissions and discharges.9. Assume hospice and palliative on-call and other responsibilities as assigned.10. Provide skilled nursing care to assigned patients/families as outlined in the plan of care. -KNOWLEDGE, SKILLS, ABILITIES:1. Possess a clear understanding of Palliative Care policies and procedures.2. Demonstrate a clear understanding of eligibility requirements for the Palliative Care Program3. Ability to maintain and track Palliative Program data.4. Excellent organizational and time management skills.5. Ability to provide excellent customer service when communicating with referral sources, patients, family members, and the Palliative Care team.6. Understand and appropriately react to emotional issues related to serious illness.7. Ability to work effectively and independently within the context of the Palliative Care team.8. Ability to work a flexible schedule.9. Participate in center/agency surveys (Licensure/JCAHO) and any subsequently required reports.10. Attend and participate in continuing education programs to keep abreast of changes in the field as well as to maintain current license/certification as required.11. -Attend/participate in all mandatory in-services.12. Honor patients'/families' rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, and right to voice complaints.13. Comply with corporate compliance program.14. Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and other issues as necessary.15. Follow established safety regulations, to include fire protection and prevention, smoking regulations, infection control, etc.16. Follow established safety procedures when performing tasks and/or working with equipment.17. Perform other related duties as necessary and as directed by supervisor.QualificationsMINIMUM EDUCATION REQUIRED: -RN/BSN -MINIMUM EXPERIENCE REQUIRED:Minimum of one (1) years of nursing experiencePalliative care, hospice, or home health care experience is preferable -MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:Current state license as a Registered Nurse (RN) in the state(s) where care is provided.Current CPR certification.Valid Driver's License and automobile liability insurance -ADDITIONAL QUALIFICATIONS: (Preferred qualifications)CHPN (Certified Hospice and Palliative Nurse) - -Job: NursingPrimary Location: North Carolina-New BernSchedule: Full-timeShift: 1st ShiftJob Posting: Jul 5, 2021, 2:57:03 PM
